How in the world did this boat/trailer disaster happen?

Is this a documented fact or speculation? I hadn’t seen any details published and would like to share any lessons learned.

I saw the tragic news the day after it happened, upon seeing the picture of the boat involved immediatly remembered the trailer/boat/truck thread but did not post anything till more details came around, as the original thread was sort of making light fun about an earlier lucky accident and this separate incident is a tragedy that could have been avoided, but a tragedy nonetheless. Below some extracts from a news site (link to full article below). Conditions can change VERY rapidly out there, there is often no time to think and little time to act when the shit hits the fan. No matter your experience, weather or whatever, always wear a PFD and have alternative safety plans in case things make a turn for the worse.

"He was not wearing a life jacket, according to local authorities (...) Hood River County Sheriff’s Sgt. Eric Wahler said it was a chaotic situation as high winds compacted the waves. Sgt. Wahler was operating the sheriff’s boat during the rescue. He said witness accounts said Shufeldt’s open bowed jet boat was in a trough between two large waves when another larger wave crashed over the stern of the boat, swamping it. Stufeldt worked quickly to help his clients get their life jackets. Passengers who were rescued were shaken up, Wahler agreed."

The fact that the captain was not wearing a PFD was noted in the local news article.
 
It's really quite unfortunate that this accident is a subset thread to the original thread.

The ONLY good to be derived from this tragedy is that perhaps somebody will be able to avoid this sad fate by taking these 'life lessons' to heart.

We all make mistakes that sometimes, under certain circumstances...are life ending.
